Independent Students

  • Your social insurance number
  • Your full mailing address where you would like to receive mail from Student Financial Services
  • The full mailing address of your parent(s), guardian(s), or next of kin
  • Lines 114, 119, 121, 129, 150, 220, and 303 of your most recent Income Tax Return, if applicable
  • Any other income Information

Information related to my private school/college/university including:

  • the schools’ name, address and telephone number
  • the name of your program of study
  • the start and end dates of your program of study
  • the start and end dates, if applicable, of your co-op work term or practicum
